DL9 Medical Assessments

A DL9 medical assessment is required by Waka Kotahi NZ Transport Agency (NZTA) to confirm that a person is medically fit to drive safely.

The medical certificate (DL9) issued following the assessment verifies that a medical or clinical examination has been completed and communicates fitness-to-drive details to NZTA. The DL9 form is included as part of the driver licence application or renewal process.

Who needs a DL9 medical assessment?

A DL9 medical assessment is required in New Zealand for individuals who are:

  • Applying for or renewing heavy vehicle licenses (Classes 2–5)

  • Applying for or holding commercial endorsements, including:

    • Passenger (P)

    • Vehicle Recovery (V)

    • Driving Instructor (I)

    • Testing Officer (O)

  • Aged 75 years or older

  • Requested by Waka Kotahi to provide medical clearance

The assessment ensures, through NZTA Waka Kotahi, that drivers and operators are fit to drive and can safely perform driving-related duties.

What does the assessment include?

A DL9 assessment is a fitness-to-drive screening, not a diagnostic medical examination. It generally takes about 15 minutes.
Depending on individual requirements, the assessment may include:

  • Review of medical history and current health

  • Vision screening

  • Blood pressure and basic health checks

  • Discussion of any medical conditions or medications that may affect driving

  • Completion of the DL9 medical certificate for submission to NZTA

What to bring

Please bring:

  • Valid photo ID

  • Glasses or contact lenses if you normally wear them

  • Details of any medical conditions or medications

Important note

DL9 medical assessments are completed to meet NZTA requirements and assess fitness to drive only.
They are not a full medical examination and do not replace care from your GP.

If concerns are identified, you may be advised to follow up with your GP or a specialist.

NZTA makes the final decision regarding license issue or renewal.

In some cases, additional information or specialist reports may be required before the DL9 can be completed or accepted by NZTA.
